Formulário do Ambulatório

O arranque para a elaboração de um formulário do ambulatório (hospitalar ou não) é um momento crítico do desenvolvimento do projecto. Tal é particularmente sensível quando não é essa a cultura dominante e quando se fazem sentir influências desfavoráveis - resistências - (quer internas quer externas às organizações)

Assim, a adopção e validação de critérios de trabalho inicial é crítica para a credibilização do conceito, para o desenvolvimento do projecto e para a sua sustentabilidade futura.

Porque tais trajectos já foram trilhados por outros, experimentando e ultrapassando dificuldades, e para que se obviem potenciais factores de resistência sem repetir erros passados, sugiro a inclusão, como anexos, de duas referências passíveis de adopção como pontos de partida para a elaboração de formulários.

Bases para a elaboração de um formulário para o ambulatório hospitalar:

1. A OMS (comité de peritos) desenvolveu um formulário modelo, validado conjuntamente pelas Royal Pharmaceutical Society of Great Britain e British Medical Association - responsáveis pela eleboração e edição do British National Formulary - e que foi publicado em 2002 (WHO model formulary, 2002).

2. O NPC (National Prescribing Centre) do NHS (National Health Service) disponibiliza discussões de workshops em terapêutica medicamentosa. Transcrevo:
